Permission

I give permission for members of the team name listed above to participate in the 1st annual Slammers and NABA High School Showcase Tournament, October 7–10, 2010.  Realizing that such activity involves the potential for injury which is inherent in all sports, I acknowledge that even with the best coaching, use of the most advanced protective equipment and strict observance of the rules, injuries are still a possibility.

Hold Harmless Agreement

In consideration of the foregoing and being allowed to participate in the NABA and Slammers Baseball activities or functions, including, but not limited to, this Tournament, its games, practices and meetings, and intending to be legally bound for myself, my heirs, executors and administrators, I and my team do hereby release and discharge the NABA, Slammers Baseball, including but not limited to, the National Organization, its officers, directors, agents, contractors and employees, and city, county and/or state organization or any other governing body that may have jurisdiction over any baseball field or other facility that I might play on, any other owners, lessors or provider of fields or facilities (whether privately held or otherwise), any umpires or umpire organizations, and their respective officers, directors, agents, contractors, employees and other players in the Association or Tournament, jointly and severally, from any and all liability from personal injury, accident, illness, disability, death, property damage or other occurrence which I or my team may suffer in any manner whatsoever arising out of or resulting from our participation in the Tournament or any NABA or Slammers Baseball activity or function held in conjunction therewith; and I and our team expressly assume all risk of my participation in these activities or functions.

Liability Wavier and Guarantee

The athlete’s safety and health is our top priority.  Every precaution possible will be taken to ensure that the risk of accident or injury is minimized, however, as with any athletic and exercise activity, there is always some risk.  Signing above indicates that you and the parents of your team understand the risks involved in participation of NABA and Slammers Tournaments and that you agree to hold harmless coaches, officers, agents, representatives for any injury the athlete may experience as a result of the athlete’s participation in these programs.

The NABA and Slammers Baseball reserves the right to refuse any player or team permission to participate in a tournament.  The NABA & Slammers reserves the right to add, delete, merge or create divisions or divisional formats based on the number of teams entered at anytime before the tournament, and will give every effort to give teams advance notice, however NABA may not be able to do so.  If a team planned to enter a deleted division, that team may play in another division or receive a refund minus your deposit, or transfer the balance to another NABA/Slammers Showcase Tournament, provided it is at least 5 days prior to check-in.  The NABA & Slammers also reserves the right to make changes to the rules or make changes in prizes and/or giveaways (but with a substitute of equal or greater value), which would then be effective upon notice to the teams participating in the tournament.
